Project Description
The Medical and Dental Record System is a LAN Based Application written in Visual Basic and MySQL. These include patient’s personal information, official health records, medical and dental history, daily transaction, medicine log and inventory, common and uncommon illness.
The school clinic is responsible in keeping these records secure and accessible. This is because, these records serves as basis in producing patients information like medical and dental history.
This will help the clinic personnel process records and keep the patients information in faster and accurate way. It will help to keep and handle essential information of the patients for their convenience. Clinic personnel can easily access and retrieve information regarding with patients records.
Menu Structure of the System
Records Menu – Health Record, Student Level, Cases, Transaction/Consultation, Dental, Medicine, Users, and About the System
Reports Menu – Patient History, Health Records, Daily Log, Dental Log
Settings Menu – Setup Theme, Date and Time Settings
System Features and User Interface
- Medical and Dental Record System Main Form
The main form of the program is the form that opens after the login process. The main form is the form in order to access the other forms of the program. The image below shows the main form of the medical and dental system.
- Medical and Dental Record System User Account Form
User account form is the form where the administrator can add, update and delete other users. Users registered in the system are the only one that can access the records of the system.
- Medical and Dental Record System Patient Information Encoding Form
Personal information of the patient and other necessary records are being encoded in the system, this is a one-time registration. Information recorded in this module is considered as confidential, thus, the administrator can only edit and update the information but the system also creates a log of all transactions.
- Medical and Dental Record System Disease Archiving Form
List of diseases are being encoded in the system for archiving and reporting purposes. The image below shows the form for the encoding process of the diseases.
- Medical and Dental Record System Medicine Information Encoding Form
List of medicines and its usage and purpose are also part of the record keeping of this project. Dispersal of medicines is being recorded for inventory purposes.
- Medical and Dental Record System Dental Examination Form
Results of dental examination are being recorded and archived, dental logs and history of every patient is also available for every patient. The form below shows the form for dental examination encoding process.
- Medical and Dental Record System Consultation Encoding Form
Consultation form is the heart of the project, every complaints, diagnostic and treatment of patient is being recorded in this form. Dispersal of medicine is also recorded in this module of the project.
- Medical and Dental Record System Patient Consultation History Form
This module can produce a printable format of the list of consultation record of every patient.
- Medical and Dental Record System Health Records Form
Health record form/module can provide the list of diseases and the number of occurrences every month, this module can also provide a graphical version of the record.

- Medical and Dental Record Personnel – they are the persons who take charge for record keeping. Since they are the ones assigned for encoding and retrieving patients record as well as statistical report needed, they must learn how to use the system.

The Medical and Dental Record System is an alternative form of record keeping to the manual process of operation, automation is more accurate, fast and reliable compare to the manual process, with the reason presented, the developers strongly recommends the utilization of the said project. A web version of the project is also part of the improvement that can be implemented.
